What Do I Need to Apply?

Most applications require:

  • Full name and address
  • Date of birth
  • Social Security number
  • Citizenship or immigration status
  • Proof of income or tax filing information

Covers health care for eligible individuals and families.

Helps families pay for childcare so parents can work or attend school.

  • What you’ll need: Name, address, date of birth, citizenship status
  • Apply: Through your Local Department of Social Services
