How to Complete and Submit the Form

These steps will help make sure your information is saved correctly and your form is received without delays.

Before You Begin

  • Use the latest version of Adobe Acrobat Reader
  • Save the form to your computer before filling it out
    (Do not complete the form in your web browser—your information may not save.)

Step 1: Save the Form to Your Computer

  • Open the form link
  • Select Save As
  • Save the file to a location on your computer, such as your Desktop or Documents folder

Step 2: Complete the Form

  • Open the saved PDF file
  • Fill out all required fields
  • Save the completed form to your computer

Tip: Some forms include a Submit button. If available, you may use it to email the form directly from the PDF.

Step 3: Sign the Form Electronically (Optional)

If the form requires a signature, you can sign it electronically:

  • Open the PDF in Adobe Acrobat Reader
  • Select Fill & Sign
  • Click Sign, then choose one option:
    • Type your name
    • Draw your signature
    • Upload an image of your signature
  • Click Apply and place your signature on the form
  • Save the signed form

Step 4: Submit the Form

Prefer to Complete the Form by Hand?

You may also:

  • Save and print the form
  • Fill it out and sign it by hand
  • Scan the completed form back to your computer
  • Email it to VRMP@dss.virginia.gov
